Medical

Evergreen offers all full-time employees and their dependents comprehensive coverage in a Preferred Provider Organization (PPO) through Guardian. Premium cost is shared between employer (75%) and employee (25%).

Rx

Through Evergreen's medical coverage provider, Gaurdian, prescription covereage is also provided. There is a $20 co-pay for generic presriptions ($40 for mail order) and a $40 co-pay for brand-name prescriptions ($80 for mail order).

Dental

Evergreen offers all full-time employees and their dependents comprehensive coverage in a Preferred Provider Organization (PPO) through Guardian. Cost is shared between employer (75%) and employee (25%).

Short-Term Disability

Evergreen provides both short-term disability coverage at no charge to all full-time employees. Short-term disability is payable for 90 days. 100% of insured earnings are paid as long as the employee can work from home and 75% of earnings are paid if the employee cannot work from home.

Long-Term Disability

Long-term disability is also provided by Evergreen. After a 90-day waiting period, 60% of the employee's insured earnings are paid to maximum monthly benefit of $6000. During the 90-day waiting period, the employee is eligeble for short-term disability benefits.

Life and Accidental Death & Dismemberment (AD&D) Insurance

Evergreen provides life insurance coverage at no charge to all full-time employees. Coverage is equivalent to their annual salary up to a maximum of $150,000. Evergreen also provides accidental death and dismemberment coverage to all full-time employees.

401(k)

After 90 days of employment, Evergreen's 401(k) Savings Plan allows all full-time employees to defer up to $15,500 of their compensation annually before any federal and state income taxes are withheld; up to $20,500 may be deferred by employees over 50 years of age.

Flex Plan

After 90 days of employment, Evergreen's Flex Plan allows all full-time employees to defer a portion of their compensation prior to withholding federal and state income taxes into Medical and Dependent Care Reimbursement Accounts from which eligible medical and dependent care expenses may be paid. Employees may deposit $2,000 into Medical Accounts and $5,000 Dependent Care Accounts annually.

Paid Time Off

Evergreen full-time employees accrue 15 days of PTO annually their first four years of employment. Evergreen full-time employees accrue 20 days of PTO annually after five years of service.

Employee Assistance Program

The Employee Assistance Program (EAP) provides free, confidential phone counseling and a referral to local providers for employees experiencing difficulties in their lives.

Tuition Reimbursement

Evergreen full-time employees are eligible for tuition reimbursement up to $1,500 per year.

Referral Bonus

Evergreen full-time employees are eligible for our referral bonus program. Employees receive $2,500 bonuses for qualified technical position referrals and $750 for qualified administrative position referrals.

Holidays

Evergreen full-time employees receive paid time off for the following holidays: New Year\'s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, the day after Thanksgiving, Christmas Eve, and Christmas Day.
